Image Challenge 22 - Identify the object


A laboratory received a diarrhoeal stool specimen from a 4 years old kid complaining of abdominal cramps and diarrhoea. Upon concentration of stool by formal-ether method and staining by modified AFB technique, the following objects measuring about 4 microns diameter were observed under oil immersion objective.

What is your diagnosis? and based on what criteria?

    It is Cryptosporidium parvum. Based on clinical complaint and the staining method and morphology of the organism. MAFS technique is used for Crypto, Isospora etc.

    Well, given the Hx of the patient and the Diagnostic modality (Modified AFB of fecal specimen) the DDs should be restricted to Cryptosporidium parvum and Cyclospora. Cyclospora is ruled out as diameter of the oocyst is around 8 -10 micron meters and here the diameter is 4 micron meters. The diameter of the oocyst of Cryptosporidium parvum is b/w 4 - 5 micron meters. Isospora belli is ruled out coz of shape of oocyst, which in this case is oval.

    Final dx : Infx with Cryptosporidium parvum.

    Cryptosporidium parvum/C. hominis infection is my diagnosis as opposed to Cyclospora as the oocysts size is 4um and not greater (cyclospora oocysts are usually is 8-10um). Also Cryptosporidium stain red in modified AFB while cyclospora could be colorless to pink or red. UV light can be used as Cyclospora oocysts show autofluorescence...

    It is the oocyst of cryptosporidium which is one of the commonest organisms causing diarrhoea and is usually stained by modified acidfast stain and is usually of the size of 4-6 microns.
